at a loss y axis stall

ill keep it short , carved yesterday 2 projects without a flaw , this am went to do another and had a y axis stall , noticed the carve ran off the keyboard side , just pryor had a clean y axis msg , so i cleaned them, lubed them and retried getting the same , ran off the keyboard side , and y axis stall , so i reflashed the card , and eliminated the boarder on the carve , retried and this time it carved all over the place ,then gave me the y axis stall , looed at the user test y axis , was 99.23 and 256 but as stated 3 times i retried it still runs off keyboard side checked bet , no issues , bearing and rollers all appear good, as i said im at a loss ,the board size was 14x14 x.0750 here is a pic of what its doing. any help would be appreciated
