Never seen anything like it. Seems the second picture of the fork is shorter than the first. My guess would be a tracking issue on the carve of the second picture. The second picture looks compressed lengthwise. Possibly the board lost touch with the tracking roller.

Slick board... I don't see any masking tape on each side.... And for this one, I would put a few strips of masking tape on the board to give it some bite to the belts...

Al , you are good, I missed the obvious, And "Use MASKING TAPE" for board skipping in the X or breaking bits

If the pattern is simply shifted in the x direction, it could just be a board measurement error. It is hard to tell from the pictures whether it is just a shift.
