Anyone have a customer order form that they would share ??

Does anyone have a blank/template that works well for taking customer custom orders ??
Looking for something that would have customer name & addy, design wanted, material to use, finish, etc.... maybe a place for a little drawing.

I can make one, but thought one of the "experts" would have one handy that had been working well for them. ( Dont want to make one up, be at a show and go..."ooops...forgot the "wood type" line"....looks unprofessional in my book)
