Plaque for church - please help

Im looking for a pattern of songbook pages with notes that i can enter the words to "Bow the Knee", for my church. Can someone pleas help me with this

If you could give an exact photo with good lighting I could easily turn it into 3d for carving for you. Maybe you want to have your church song book open to the page and then a sign above it saying "bow the knee". Or maybe a simple scetch to give an idea of what you want.
