Z drive stall error 298

Does anubody familiar with the Z drive stall error 298??

Project starts well and somewere in the midle bits start working in the air.:(

Dust in the Z Encoder

Dust in the Z Belt

Bit Plate did not touch due to dirty rails or belt or cogs.

Loose Bit in holder
