I need the "Cover Template"

I received an e-mail from Joe Lovchik about submitting projects to the pattern store. It had several attachments. I could not open the "Cover" template (in Word format) attachment. All the other files were OK. I have e-mailed Joe, sent a tech support request and called LHR sales to forward my message, and received no response. Does anyone have that file that you could send me?

Thanks, I saved it then tried opened Word and opened it just fine. Joe just resent me the one he sent previously, but it still will not open on my computer. I will have to E-mail him and tell him not to re-resend it. Thanks a bunch.
