Here are some pics of the parts I'm using in my latest boxes. Our local Office Depot is having a going out of business sale and I scored about 2 dozen of these bulbs in various sizes for ~$2.50/ea.
The candlelabra bulb has a frosted glass cone on it which diffuses too much light so I used my dremel with a cut-off wheel and cut through at the point shown then pried/cracked it off with a screw driver. It comes with an adapter to fit a standard socket.
The rest of the parts are a light socket that plugs into an outlet, double outlet, shallow outlet box, replacement lamp cord (not shown).
I cut a 1/2" hole in the back of the litho box to feed the cord through. Drill 2 holes in the outlet box to mount it to the bottom of the litho box off-centered so the bulb ends up in the middle.
